Cardiogenic shock portends a dismal prognosis. Without traditional invasive monitoring available in ICUs and immediately available comprehensive echocardiography, it can often be a challenge to recognize in the ED. Furthermore, different phenotypes of cardiogenic shock require vastly different treatment and management. Early recognition is key for improving patient mortality. This talk overviews pitfalls in both the recognition and management of cardiogenic shock. Salient points will include discussing erroneous assumptions about the physical exam, misinterpreting laboratory values, utilizing point of care ultrasound to rule out deadly and time sensitive causes of cardiogenic shock and avoiding the management errors with inotropes, antiarrhythmics and other treatments. We will also discuss when it is crucial to consult cardiology and cardiac surgery for interventions and escalation to mechanical cardiac support.

Utilize the physical exam to differentiate between right heart failure and left heart failure.
Perform point of care echocardiography to assess for etiologies of cardiogenic shock.
Review indications for emergent consultation and escalation to mechanical cardiac support.
